Serenity's care extends beyond the physical well-being of our patients; we provide emotional, psychological, and spiritual support to both the patient and their loved ones. Who is eligible for hospice services?

Hospice care is intended for individuals with a terminal illness or a life expectancy of six months or less, as determined by a physician. Eligibility for hospice care is not limited to a specific age or diagnosis but is based on the patient's overall health status and prognosis. This includes individuals with advanced stages of diseases such as cancer, heart failure, chronic obstructive pulmonary disease (COPD), Alzheimer's disease, and other life-limiting conditions.

The focus of hospice care is on managing symptoms, providing pain relief, and enhancing the patient's quality of life rather than seeking curative treatment. It is essential that the patient and their healthcare team agree that curative treatment options are no longer beneficial or desired. Hospice care also extends its support to the patient's family members, recognizing that they too experience emotional, practical, and spiritual challenges during this time.

Eligibility for hospice care is determined through a collaborative assessment involving the patient, their primary care physician, and the hospice care team to ensure that the individual's needs align with the philosophy and services provided by hospice care.

How can Serenity provide extra financial help for you?

Our services include medications related to your hospice diagnosis, hygiene and incontinence supplies, and medical equipment like oxygen supplies, walker, wheelchair, hospital bed, etc.

What will your medical team look like?

Your care plan will include a physician, nurse, medical aide, social worker, chaplain, and volunteers. We provide 24/7 support along with weekly nursing and medical aide visits; and as needed visits from your social worker, chaplain, and volunteers. 

What are the goals of hospice?

Empower patients to maintain their independence by offering personalized support that focuses on their individual needs, preferences, and desires.

Alleviate mental, physical, emotional, and spiritual distress, to ensure that patients experience comfort, peace, and dignity throughout their end-of-life journey.

Provide guidance, counseling, and bereavement support to the patient and their family to help navigate the emotions related to the end-of-life experience.
